COMMUNICATION OF PRIVACY AND DIGNITY POLICY

To ensure Clients understand the subject matter of this Privacy and Dignity Policy in a manner which is
responsive to their needs and in the language, mode of communication and terms that the Client is most likely
to understand, National Care Australia will:

The personal information that National Care Australia collects from a Client includes their:

National Care Australia only collects sensitive information where it is reasonably necessary for National Care
Australia’s functions or activities and either:

information including health and medical information and information relating to their disability and support

the privacy and dignity of each Participant and, in particular, their sensitive information.

WHY DOES NATIONAL CARE AUSTRALIA COLLECT PERSONAL INFORMATION?

The main purposes for which National Care Australia collects, holds, uses and discloses personal information

CAN I WITHDRAW OR AMEND MY CONSENT TO THE USE OF MY PERSONAL INFORMATION?

A Client may withdraw or amend their consent to National Care Australia using their personal information at
any time by written notice to National Care Australia. The nature of the business carried on by National Care
Australia means that, generally, it is not possible for National Care Australia to provide services or supports to
Clients or otherwise deal with individuals if a Client withdraws or amends their consent.

Legislation, Rules, Guidelines and Policies apply to this policy and supporting documentation as set out in the
Legislation Register.

INCONSISTENCY

If and to the extent that the terms of this Policy are or would be inconsistent with the requirements of any
applicable law, this Policy is deemed to be amended but only to the extent required to comply with the
applicable law.
